Cruz Davis Family and Cosmetic Dentistry is helping bridge this dental health disparity through our low-cost Cruz Club dental membership program. 

Cruz Club members receive their preventative dental care for free, including dental cleanings, x-rays, doctor exams, and fluoride treatments. Members also receive a 10% discount on all additional dental treatments.

Patients pay a low monthly membership fee that includes the following COMPLIMENTARY annual preventative dental services:

  • 2 Professional Dental Cleanings (patients with healthy gums)
  • Needed Recare X-Rays (determined by our doctor)
  • 2 Doctor Recare Exams during Cleanings
  • 2 Fluoride Applications 
  • 1 Emergency Exam with Diagnostic X-Rays
  • 1 Comprehensive Exam (during new patient appointment)
  • 3-4 Periodontal Maintenance Cleanings (Periodontal Plus Add-On subscribers)

Our Membership Subscription Fees 

CRUZ CLUB is a private subscription service that automatically renews and charges your stored payment method every month unless cancelled by you or us.

  • There is a one-time Activation Fee per new membership.
  • Activation Fee is $75 per individual membership.
  • Activation Fee is $250 for the Family of Four Membership.
  • Activation Fee is $275 for the Family of Five-or-More Membership.
  • Members may cancel any of their subscriptions at any time.
  • No refunds or prorations will be provided.
  • A reactivation fee of $200 will be required if membership is not current or the dues lapse by 28 days.
  • Currently there are no Activation Fees for add-on subscriptions.
  • Periodontal Plus add-on subscription is required for members diagnosed with periodontitis (disease affecting gum and bone).
  • Doctor determines patient’s gum health during comprehensive exam.
  • If doctor diagnoses CRUZ CLUB member with periodontitis, we will add the Periodontal Add-on subscription to that patient's membership, effective immediately. 
  • Patients must abide by all Cruz Davis Family and Cosmetic Dentistry policies, including a  $100 per hour broken appointment fee, which our office will automatically charge to your stored payment method when applicable.
  • Monthly dues, terms of service, and dental service fees may change at any time.

Please sign up each individual for an appropriate Membership below. Household family units of four or more are encouraged to sign up for one Family Membership.

Periodontal Plus add-on subscription is required for members diagnosed with periodontitis (disease affecting gum and bone). If doctor diagnoses CRUZ CLUB member with periodontitis, we will add the Periodontal Add-on subscription to the patient's membership.

FAQs About Cruz Club

  • When can I start using my membership?

    Our secure system takes 3 business days to process new memberships. Please contact our office or visit our scheduling page to book your first complimentary appointment.

  • Which forms of payment do you accept?

    Our system currently accepts Visa, Mastercard, Debit Cards, and ACH transfer from your bank account.

  • How much can I expect to pay after each preventative care appointment?

    NOTHING! Your comprehensive preventative dental care is complimentary with your CRUZ CLUB membership. Just remember to make your appointments, and make sure to keep your subscription “current” or you will lose all special membership benefits.

  • How do I schedule an appointment?

    Simply visit our online scheduling page and make a complimentary preventative care appointment for yourself or your household family members, or contact our office for one of our team members to help schedule you.

  • How does my special CRUZ CLUB member loyalty discount work?

    In addition to complimentary preventive care, CRUZ CLUB members receive 10% off other treatments provided by Cruz Davis Family and Cosmetic Dentistry.

  • What happens if I miss an appointment?

    It is the patient’s responsibility to make, keep, or reschedule their appointments with our office. Please see our policy regarding no-shows and last minute cancellations.

  • Can I add members to my account?

    Yes, your entire family can join with individual memberships or one Family Membership.

  • What is the difference between an individual membership and a Family Membership

    Most membership subscriptions are for one individual patient, whereas Family Memberships are designed for a household of at least four family members together.

  • What fees should members expect to pay in addition to the monthly subscription fee?

    Each membership requires one Activation Fee to join. This set-up charge is in addition to the first month’s membership dues.

  • How do I pay for my membership?

    Memberships are automatically charged to your stored payment method on a prepaid monthly basis.

  • Are there penalties for cancelling my membership?

    No, there are no penalties for cancelling a subscription.

  • If I cancel my membership, will I receive a refund?

    No, no refunds or prorations will be issued.

  • When will I have to pay a Reactivation Fee?

    Members will only pay a Reactivation Fee if membership is not current or has lapsed by at least 28 days.

  • When am I considered a CRUZ CLUB member?

    You are a CRUZ CLUB member entitled to all membership benefits when you have an active subscription and are current on your dues.

  • How can I change, update, or cancel a subscription, modify billing information, or check the status of my account?

With our standard plans, the Employer chooses a monthly contribution amount of either $15 or $30 per participant, and the Employee pays the remainder of the membership. The business will also choose whether or not to extend subsidized dental benefits to employee family members.

  • How can I sign up my business for CRUZ CLUB Biz?

    Small business owners can quickly enroll their company in CRUZ CLUB Biz by using the three buttons toward the top of this webpage: 1. Employer Inquiry  2. Add Payment Method  3. Add Employees. We will also co-sign an agreement stating the terms of your company’s plan participation.

  • What happens after I sign up my business for CRUZ CLUB BIZ?

    After you enroll your company in CRUZ CLUB BIZ, please add participants to your plan, using the Cruz Club webpage button 'Add or Delete Employees or Family from your Plan.' You can always make changes to your participant list, using the same button.


    Once you have added participants to your plan, our team will process your Biz membership within three business days. We will then Email you a welcome letter to share with your plan's participants. Each participant should then visit the EMPLOYEE section of the Cruz Club webpage and sign up for their subsidized membership to begin taking advantage of all the Cruz Club member benefits.

  • Which forms of payment do you accept?

  • How much will CRUZ CLUB Biz cost my business?

    Contribution amounts per Employee participant are $30 / month or $15 / month. The employer will choose its contribution toward the employee’s monthly membership fee, and the employee will pay the remainder. 


    Your company will also choose whether to extend subsidized CRUZ CLUB benefits to employee family members. The employer can enroll or remove employees and family members from the company’s CRUZ CLUB Biz plan at any time by notifying us in writing.

  • How will my company be billed?

    Your payment method will automatically be charged each month. The charge will reflect your CRUZ CLUB Biz company contribution, multiplied by the number of participants that your company is covering at the time, minus any prorations for participants added mid-cycle.

  • How do I add or delete employees / dependents from our CRUZ CLUB Biz plan?

    To add our remove employees from your company's Cruz Club Biz plan, you can use the corresponding button near the top of this page, or simply send us an Email or message from within our website.

  • What is the difference between CRUZ CLUB and CRUZ CLUB Biz?

    CRUZ CLUB is a private membership program that our Gainesville dental team designed to help ‘fill a gap’ in dental healthcare access for the uninsured--or the insurance-free, as we prefer to say. CRUZ CLUB is designed for individuals and families, with a concept that includes affordable subscription memberships and automatic billing for a simplified, efficient and enjoyable dental experience.


    CRUZ CLUB Biz is designed for workplaces to be able to easily and affordably offer dental benefits to their employees, and optionally to employee family members, by paying a part of the monthly membership dues.

  • If I do not extend dental benefits to employee family members, can employees still enroll family members in CRUZ CLUB?

    For sure! If your company chooses not to extend Cruz Club Biz benefits to employee family members, the family members can still join Cruz Club. The employee would join through the special CRUZ CLUB Biz Employee sign-up link and pay the subsidized monthly amount, whereas family members would sign up using the main CRUZ CLUB links and pay the full monthly subscription dues.

That means that your employer is offering dental benefits to you by making monthly contributions to a CRUZ CLUB membership. CRUZ CLUB is a dental membership program exclusively at Cruz Davis Family and Cosmetic Dentistry, your dentist in Gainesville FL, where club members enjoy complimentary preventive dental care, and a loyalty discount on other treatments at our office.

Employee Cruz Club Biz Enrollment

Please enroll in your company’s correct CRUZ CLUB Biz plan, according to the details of your welcome letter. Please note that there is a one-time $75 set-up fee per individual membership, or $275 for the Family Membership.

    Please have your employer set up a company plan. Once your employer enrolls your workplace and sends us a list of participating employees, you and your coworkers can quickly sign up using the above special link, and then make your first complimentary preventive care appointment with our dental office.

  • What happens after I sign up my business for CRUZ CLUB BIZ?

    Absolutely! If your employer elected to extend dental benefits to employee family members, please sign them up using the same special Cruz Club Biz Employee link; if not, simply sign them up with an individual Cruz Club subscription.
